We take PPO plans (no HMOs) from:

We also take Medicare and we offer reduced self pay fees for Medicaid patients.

What if I don’t live in Illinois? I’m an out of state patient?

**You must be physically IN ILLINOIS for us to use your insurance plan for the visit **

Otherwise we offer SELF-PAY rates for out of state patients doing Telemedicine. See below

What are your Self-pay rates?

New consultation visit fee: $400.00 (Telemedicine or In-Person)
Follow up visit fee: $150.00 (Telemedicine or In-person)

COSMETIC CONSULTS ARE FREE

“Cosmetic consults are FREE. This includes consults for:

Phallofill Girth Enhancement/Penis Fillers
Labial Puff
Labiaplasty
Facial Injections and Fillers
G-spot amplification
Scrotox
O-shot
PRP

Lastly, we understand that things come up and may need to reschedule your visit. Please notify us immediately if you need to reschedule. We do charge a $50 no show and late cancellation charge if you cancel within 48 hours of your visit.
