We are located in the Napleton Ashton Martin dealership building, near the intersection of Clark and Maple in Chicago’s historic Gold Coast neighborhood.
Covered garage parking is available at $6.50 for 3 hours in the Gold Coast Galleria Garage, attached to the building.
Enter this location at 1014 N Clark St. It is located on the west side of Clark St., between Maple St. and Oak St. The entrance is marked by a black ‘Self Park’ sign with white lettering. Park anywhere that is NOT reserved and take your parking ticket with you, so we can validate it in our office for that $6.50 rate. Please be aware that parking garage payment is by credit card only upon exiting the garage.
Getting to Suite 310: Please remember your parking spot number and floor where you parked. Follow the signs to the Elevator in the parking garage. Take the Parking Elevator DOWN to the 1st floor. Once on the 1st floor, get off the Parking Elevator and head straight across to the Building Elevator directly in front of you. Take the Building Elevator UP to the 3rd floor. Take a left upon exiting the elevator on the 3rd floor. Our suite 310 is directly at the end of the hall.

We are located in the Co-Optim building at this address. Parking is free in any available spot in the parking lot. Enter the building and walk straight to the elevators on your right. Take the elevator to the 3rd floor. Take a left upon exiting the elevator on the 3rd floor. A receptionist should greet you in front of the expansive waiting room.

Video Visits are conducted through our patient portal app (when you book a Telemedicine appointment, you will get emailed instructions on how to log into our patient portal and download the app. This will allow you to connect easily to your video visit at the scheduled time).
